Monday to Friday 9AM-5PM (subject to change based on needs of the company) Position Overview:
A patient advocate serves on the front line of the company.
The patient advocate spends his/her day in various nursing home facilities, and, as such, this job does require travel.
A patient advocate develops strong relationships with nursing home patients to identify customer's needs and assure that the company's values are reflected in every encounter with clients, families, and our referral sources.
It is also critical for the patient advocate to cultivate strong relationships with various nursing home employees.
Responsibilities:
Visit patients in nursing home facilities who are in need of home care services and educate them on personal care services.
Build, maintain, and create strong relationships with patients and their families Conduct assessment to determine if patients will be eligible for long term care Able to identify interested patients and assist them in determining the best home care options for them (PCA, CDPAP or private pay).
Assist patient with enrollment into the company through authorization Collaborate with internal departments to facilitate patient need and fulfillment Provide progress reports to patients as well as management Resolve patient issues and complaints Other assigned duties as directed by management The salary range for this position is $50,000- $55,000 (plus incentives) commensurate with experience.
